Appointments
 We see all patients on an appointment basis, and ask that you call in advance so that we may reserve time for you. The office phone number is (831) 649-3337. We make every effort to honor all of our time commitments and ask that you do the same for us. Please provide us with at least 24 hours advance notice for any appointment changes. This will enable us to better accommodate another patient.
Office Hours
The Monterey office is open on the following days:
Monday through Friday: 8:30 am - 5:00 pm
The San Jose office is open on the following days:
Monday through Friday: 8:30 am - 5:00 pm; Saturday (San Jose only): 8:30 am to 3 pm
Emergency Care
 In the event of an emergency, call our regular office telephone number and follow the instructions prompted on our message.
Telephone Calls
We encourage you to call with questions you may have concerning you health care problems, however, it would be unfair to other patients if the doctor were to disrupt treatment to answer every phone call. Our staff is trained to answer most questions.
Fees, Payments & Insurance
We participate in several insurance plans. We suggest you call our office to see if the procedure you are considering is covered by your insurance. For those procedures not covered by your insurance, Visa, & Master Card are accepted. Financing is also available for aesthetic procedures through Care Credit. |

Patient Forms
 The following form can be downloaded to your computer and printed at home. By filling out this form ahead of time, you will save a significant amount of time during your visit. Please be prepared to provide this information to our office along with your current medical insurance information. If you have any questions when filling out this form, please do the best you can and our staff will assist you with your questions on the day of your visit.
